| After 30 years, it is with mixed emotions that I am hereby notifying you of my decision to retire from the practice of medicine and to close my practice at Tyrone Family Medicine effective May 30, 2025 (the “Closing Date”). After the Closing Date, I will no longer be able to provide your medical care. This has been a difficult decision as I have enjoyed long-term care relationships with my patients, in some cases multigenerational. I very much appreciate the support from our loyal patients. In an ever-changing healthcare landscape, the challenges of self-employment have risen dramatically, and we have been unable to locate a suitable entity to take over the practice. Your health and well-being are of great importance to me. Therefore, I encourage you to choose another provider as soon as possible to ensure uninterrupted care. If you are not acquainted with a local physician, we can provide a sample of local providers, or you may use your insurance company’s provider directory. Piedmont Healthcare also has a physician finder link that may be helpful to identify other local medical offices that you may contact to establish care. Your medical records are confidential, and records can be released to you or your new provider only with your written permission. If you identify a new physician prior to the Closing Date, you may contact my office to obtain copies of your medical records. In accordance with Georgia law, there will be a nominal fee to cover copying and transmittal costs. After May 30, 2025, your records will be transferred to and maintained by Morgan Records Management for a period of ten (10) years, in accordance with applicable confidentiality and security standards. If you wish to have a copy of your records after the Closing Date, you may submit a written request to Morgan Records Management. Requests should be directed to: |
